To apply for B-W So You Want to Be... summer academic institutes, please print and complete the following application and nomination forms.

Please read and follow all instructions carefully. Note the the application packet requires three items, all of which are to be sent in one envelope, to the address above:
- Application form
This form is to be completed by the student including an essay answering specific questions about why he/she wants to participate in the institute and what they hope to achieve. - Nomination Form - Teacher
After first filling out and signing the FERPA section on this form, the student should ask a teacher who has had him/her in class to complete the nomination form in a sealed envelope, signed across the flap. - Nomination Form - Other
The student should first fill out and the FERPA section of the form before asking a leader who knows the student personally to complete the nomination form on the student's behalf. Examples of people a student might ask to fill out this form would include a former or current coach, pastor, youth leader, music teacher, etc.
